Nationwide Health Plans offers health insurance plans in California and Ohio only, including individual health insurance products as well as offerings for employer groups.
Nationwide Family Plans
For individuals and families in California and Ohio, Nationwide offers three different PPO health plans
* LifeStyle Series Health Plan- These high deductible plans, between $2,500 and $5,000, offer 100% coverage after the deductible. Benefits are comprehensive, including regular doctor visits and emergency/hospitalization services. As an added bonus, the deductible is waived for your first four doctor visits.
* PPO Choice Saver Plus- This PPO plan has similar benefits to the LifeStyle series plans, but includes the option of using a health savings account to finance your health care.
* PPO Choice 25- This plan gives you a range of deductible options, from a low $750 to $5,000. Benefits are similar to both PPO Choice Saver Plus and the LifeStyle Plans.
