Applicants may choose a benefit package with or without a limited prescription drug benefit.
Empire's Direct Payment HMO/POS plan offers you the benefits of both a traditional HMO (Health Maintenance Organization) for in-network services and the additional freedom to obtain out-of-network POS (Point of Service) medical services. You choose your Primary Care Physician (PCP) from within the HMO network, who then provides or coordinates most care.
Empire's Direct Payment Health Maintenance Organization (HMO) plan includes benefits for "in-network" services only. Your chosen Primary Care Physician (PCP) provides basic and preventive care, gives you referrals to specialized care, maintains your medical history, arranges admissions to hospitals and pre-certifies special services - all within the network. Empire's Tradition PLUS Hospital plan covers your hospital needs-and the hospital needs of your covered dependents-even when you are far away from home. Your Empire Blue Cross Blue Shield Identification (ID) card is accepted at over 5,700 hospitals and other facilities nationwide, assuring paid-in-full inpatient hospital benefits for covered services.
