BCBSAZ Blue Preferred Individual Health Insurance Plans

BluePreferred^® is a preferred provider organization (PPO) plan with deductible choices ranging from $250 to $5,000. With this plan, you pay fixed copays for visits to most PPO doctors (without having to meet your deductible first). Your out-of-pocket costs are lowest when you use providers in the PPO network, but you also have the flexibility of using providers outside this network for most services. Plus, when you travel you have access to a national network of PPO providers from independent Blue Cross and/or Blue Shield plans throughout the country.

BluePreferred Saver is a high-deductible PPO plan that meets federal requirements for use with a health savings account (HSA).* There are two deductible options available: $5,000 and $2,600. All services count toward your deducible first, except preventive care. Your out-of-pocket costs are lowest when you use providers in the PPO network, but you also have the flexibility of using providers outside this network for most services. Plus, when you travel you have access to a national network of PPO providers from independent Blue Cross and/or Blue Shield plans throughout the country. *You must satisfy certain criteria to be eligible to open a health savings account. Check with your tax or legal advisor regarding whether you satisfy these criteria.

BluePreferred Basic is a lower premium, high-deductible preferred provider organization (PPO) plan with deductible choices of $5,000 to $10,000. Most services are covered with a coinsurance percentage, after you meet your deductible. Your out-of-pocket costs are lowest when you use providers in the PPO network, but you also have the flexibility of using providers outside this network for most services. Plus, when you travel you have access to a national network of PPO providers from independent Blue Cross and Blue Shield plans throughout the country.
