BCBSAZ Individual Health Insurance Plans
Blue Optimum Plus - a preferred provider organization (PPO) plan provides copays to cover in-network services such as urgent care and office visits.
BlueValue Plus - a PPO with a mix of copays and coinsurance cost sharing plans to provide flexibility.
BlueEssential Plus - low cost PPO with basic coverage and copays for up to three physicians visits per year.
BlueBasic Plus - a PPO with copays for all physician visits and coinsurance after the deductible.
BluePortfolio Plus - high deductible PPO for use with a health savings account with three deductible levels.
All BCBSAZ individual plans include physician office visits, hospital benefits, prescription medications, routine vision exams, routine physicals and preventive screenings, and child care with routine immunizations.
