As the state's largest local health insurer, Blue Cross Blue Shield of Arizona (BCBSAZ) has been dedicated to offering health insurance products or services to Arizonans since 1939.

BCBSAZ Individual Health Insurance Plans

BluePreferred^® is a preferred provider organization (PPO) plan with deductible choices ranging from $250 to $5,000.

BluePreferred Saveris a high-deductible PPO plan that meets federal requirements for use with a health savings account (HSA).

BluePreferred Basic is a lower premium, high-deductible preferred provider organization (PPO) plan with deductible choices of $5,000 to $10,000.

BlueClassic^® is an indemnity plan that gives you complete freedom of choice with regard to your hospital or physician; plus, you can realize cost savings whenever you use a provider that participates with Blue Cross Blue Shield of Arizona or other independent Blue Cross and Blue Shield Plans nationwide. There are six deductible options available, from $250 to $5,000.

Blue Select^® is a health maintenance organization (HMO) plan that is ideal for those who don't want to bother with deductibles. You simply make a co-pay when you receive most covered services. Unlike some HMO plans, Blue Select does not require that you obtain a referral from a PCP for visits to network specialists.
