Iowa Health Insurance for Children
The Hawk-I is a program that provides health care coverage for uninsured children in the state of Iowa.
Hawk-I provides limited health insurance benefits for no more than $20 per month, and some qualifying families pay nothing at all.
To qualify children must: 1) reside in Iowa, 2) be under age 19 3) have no other health insurance 4) be a US citizen or qualified alien 5) meet the income limits established by Hawk-I program 6) ineligible for Medicaid 7) not be a dependent of an employee of the state of Iowa.
Qualified participants can choose from health plans offered by Wellmark Health Plan of Iowa's Blue Access Plan or Americhoice hawk-i from United Health Care of the River Valley
